Provides a wide variety of services for older adults to keep seniors in their home, case managers, social workers, all provide guidance and education. Medicare or Medi-cal Case Managers to search options and programs, wait list.

Health Services: Dental, Support Groups, Respite Care, Senior Non-Emergency Medical Transportation, Mobile Eye Clinic.

Dental Clinic: (714) 702-1584 Eligibility: Age 5yrs+, offered to families with no access to dental care.

UCLA Mobile Eye Clinic: 2-3 times per year; free eye exam for ages 8 yrs old and older.

Miscellaneous: Meal Delivery, Free Tax Filing, Financial Literacy, Computer Literacy and Translation, Document Preparation & Case Management (Supplemental Security Income, Social Security, Medicare, Medi-Cal and Housing Authority).

Eligibility: Uninsured or underinsured. Call for enrollment assistance and appointment.

Bring proof of OC residency (utility bill), income verification (two most recent pay stubs or unemployment check, income tax, bank statements, SS care). Non-US citizens encouraged to apply.

Pharmacy Services: Medication Therapy Program: One-to-One for medication management and integrated medication management. Patient Assistance program application.

Behavioral Health: Individual, Family, and Group Therapy. Medication evaluation.

Case Management: Multidisciplinary team that provides counseling, referrals, follow up, discharge planning, and eligibility assistance.

Food Pantry: distributed at Second Harvest Food Bank every Monday from 9a-4pm. Bring Photo ID and proof of OC residency.

Financial Assistance: Necessities such as utilities, rent, transportation. Photo ID, proof of OC residency, and documentation needed to support request.

Homeless Services: Emergency shelter, bus passes, food pantry, detox, residential treatment, nicotine patches (harm risk reduction).

Legal Aid Services: Every 2nd and 4th Tuesday from 9am-11pm. For more information on services provided call Public Law Center at (714) 541-1010.

CA Lifeline: Tuesdays and Thursdays 9am-3pm. Discounted home and cell phone services.

Mental Health: Every Friday 9am-10 am. Advocacy and education.

Disability Support and Advocacy: Services include transition from institutions support, arrangement for ancillary equipment and adaptive/assistive technology, housing, independent living skills, help with approval process for IHSS. Eligibility: Must be OC resident.

Personal Assistant Service (Registry of Caregivers): Aid with personal care and housekeeping. Eligibility: requires assistance with ADLs, those looking for caregivers with IHSS and those able to pay for caregivers. Apply by calling (714) 621-3300 or complete online form.

Shared Housing: Eligibility: 18+yrs, monthly income of $900 or more.

Offers a variety of services ranging from case management, assistance with housing, referrals to community resources, medication management. Best to access services online for more information.

Eligibility: Age 55+ yrs. Must live in PACE service area, be certified by the state to need Nursing Home Level Care, be able to live safely in the community. Program for seniors who are safe at home but at high risk for requiring institutionalization. Through CalOptima. Dual eligibility (Medi-Medi).
