Age-Friendly Health Systems

Transforming clinical training environments into integrated geriatrics and primary care systems to become Age-Friendly.

To increase the quality of care for older adults, UCI GWEP trains a wide range of medical and community service providers, as well as older adults and families on the 4Ms framework. All of our training provided to primary care providers, health professions students, older adults, patients, and caregivers focus on the 4Ms framework.

Via our UCI GWEP efforts, we have established a Geriatrics Systemwide Committee representing our ambulatory, inpatient, and emergency departments at UCI Health to help achieve the highest Age-Friendly Health System status from the Institute for Healthcare Improvement – Committed to Care Excellence for Older Adults. 

UCI GWEP works with its primary care, academic, and nursing home sites to guide them in their journey to join the age-friendly health systems movement. This is achieved through mentoring and training.

The Age-Friendly 4Ms: Medication, Mentation (mind), Mobility, & What Matters Most guide all of our education offerings.

Age-Friendly Geriatrics Tele-ECHO Series

Join our virtual CME education series where all of our training topics fall under the 4Ms framework. Sessions are led by our geriatrics faculty team or a guest presenter and are geared towards primary care providers. CME credits available.
