UC Health Care Planning Study

Advance Care Planning or ACP is the discussion of end-of-life preferences between the patient and their physician. ACP can help patients receive the care that matches their values. An Advance Direction is a legal document where patients specify their care preferences.

The UC Health Care Planning study is examining different ways of helping patients complete Advance Directives. The results may help other clinics determine how to encourage advance care planning. Ultimately, the goal is that this will improve end-of-life care for patients.
